Make a complaint

If you have followed our communication flowchart and wish to make a formal complaint please follow the steps in our Complaints Procedure.  You will be asked to complete a complaints form (Appendix 1) giving some information about your complaint.  If you would like us to send you a copy of the form please complete the contact form below.
